Care Pathways

Our hospitals provide a wide range of treatments across inpatient, outpatient and community settings delivered by an experienced multidisciplinary team committed to innovative, patient-centred care and quality outcome

Inpatient Services

We provide a range of inpatient treatments, services and therapies that are designed to treat a variety of mental health conditions – with our treatments often specialised specifically for both the diagnostic category and the age cohort.

Our hospital inpatient services are designed for patients that need a higher level of support from their treating psychiatrist and the multidisciplinary team of mental health experts.

Day Programs

Our day programs are evidenced-based and can provide continuity of care and support after inpatient hospital admission or as an alternative to those who do not require inpatient admission.

The programs have been designed to provide support and the skills needed for patients to stay well

Community

Our acute community service is an outreach, mental health home visit service provided by experienced clinicians and mental health nurses.
It serves as an alternative to hospital admission, or as part of the discharge plan after a hospital stay. It addresses a definite need to support people out of hospital, prevent relapse and help to assimilate to life in the home and community

Community services include:

  • Psychoeducation
  • Relapse prevention planning
  • Skills training in management of symptoms and medication
  • Exposure therapy
  • Brief therapeutic interventions
  • Occupational therapy assessment for the home environment
  • Exercise physiology transition of hospital exercise plan to the community
  • Social work assistance with financial, legal and housing issues
  • Vocational support
  • Community engagement
